of-pocket expenses. We are also able to provide several
benefits to you and your family, decrease your taxes, and
possibly increase your pay - all without any additional cost
to you.
All employees that qualify for this new Wellness Program have been automatically
enrolled. If you wish to waive participation in the Kingston Health & Wellness Program
and to decline receiving all of the benefits provided to you at no net cost, then please
fill out the Declination Form in its entirety.
Please note that your opt-out period closes within 30 days of the start of your
enrollment/education campaign.
